Individual accessible houses and apartments designed in a contemporary style for clients and their families wishing for full independent living with the access to all the services and facilities on offer from the rest of the development.
The apartments and houses will have a concierge who can co-ordinate a full range of services that can be purchased to meet the needs of each individual. People living in the transitional accommodation will be able to access specialist care, support, therapy, dining, the health club, information and other services from the community hub.
We aim to provide a seamless integration of services throughout an individual’s pathway. Too often these services are not integrated and at times of transition the individual will often need to adjust to new therapists or carers. While this is sometimes inevitable, we aim to make the process easier by working closely with providers of specialist care services, vocational rehabilitation and other providers so that this support can be planned for and in some cases begin, prior to discharge from the rehabilitation facility.
