Rehabilitation is a treatment, or a range of treatments designed to facilitate the process of recovery from injury, illness, or disease to as normal a condition as possible. Neurorehabilitation pertains to rehabilitation carried out to aid recovery from a nervous system injury and is typically carried out a number of professionals with specialised skills and experience, appropriate to the client or patient group.
Rehabilitation will often involve a range of activities undertaken to restore some or all of the persons's physical, sensory, and mental capabilities that were lost due to injury, illness, or disease with the support of various professionals. The rehabilitation process will typically involve a number of specialist professionals who work in different way with a client such as physiotherapist.
Neurological rehabilitation refers to rehabilitation provided to people with neurological conditions such as acquired brain injury and long term neurological conditions. At Chase Park Rehabilitation Suite the rehabilitation provided by the multidisciplinary team is overseen by a consultant in rehabilitation medicine.
Initial Prediction of Rehabilitation Potential
Prior to admission and as part of the necessary process of collecting information that we require in order to safely and appropriately offer a placement we provide our Rehabilitation Potential Report. This is compiled by our experienced assessment team and sets out the likely goals, areas of intervention and the outcomes thought likely. Even at this early stage we will suggest timescales and invite a review and discussion of more specific goals and timescales after an initial assessment and treatment period.
This initial prediction is based on the information gleaned from the client and their family, any treating clinicians, records we are given access to and the experience of our team members.
Specialist care and therapy is provided by a skilled and specialised team of Nurses, Carers, Occupational therapists, Physiotherapists, Speech and Language therapists and Clinical Neuropsychologists supported by experienced Therapy Assistants and our Social Opportunities Team.
In reality it is difficult to separate the individual disciplines as the team is truly interdisciplinary and seamless working with nursing and care staff achieves therapeutic approaches 24 hours per day with appropriate use of resources where available and the avoidance of duplication in assessment and intervention.
After discharge our specialist therapy team are able to follow the client out into the community to provide continuity of rehabilitation.
