Integrated Housing Pathways
Here at Keiro we have changed our model recently to incorporate a partnership approach to provision of integrated health, housing and well being services. We are committed to working in partnership with other organisations to facilitate pathways which enable our neuro rehabilitation clients to make the transition from hospital to home. We believe that in order to give our clients the best opportunity to move back to the community and into independent living we need to have meaningful partnerships in place. People with brain and spinal injuries and those with long term neurological conditions who are either recovering from a traumatic episode or a period of ill health need integrated care pathways to enable them to regain and retain their independence.
I read a recent article by the National Housing Federation report which articulates the benefits of healthcare and housing providers working together to help people to maintain their independence. The report also shows the amount of money that can be saved by commissioners of services by helping people to access the right kind of housing. Accessible transitional housing is something we feel is important to for the clients that we provide services to (neurological conditions, stroke, acquire brain and spinal injuries) because of the physical disabilities that they have and also because it can be difficult to assess whether a person will mange to make the move straight from a hospital or rehabilitation setting back to the community.
This infographic on our website illustrates the way that we think clients have the best opportunity to make the transition from hospital back home. As you can see transitional and long term housing is a major aspect of the pathway, but there are also coordinated social, leisure and information services which enable people to move to independent living.
We recently held a conference in Middlesbrough with our housing partner Erimus where we launched the range of services which will be providing from 'The Gateway'. We will be sharing some ifollow up information about the conference in due course. In the mean time here a couple of pictures from the event.
